Incident notification

ONR is the enforcing authority for legislation that places a duty on specific individuals and/or organisations to notify ONR of incidents that have occurred. This includes relevant incidents associated with ONR’s purposes: nuclear safety, security, transport of radioactive material, safeguards, and nuclear site health and safety.  

Details of duties, related legislation and ONR’s notification procedures, are described in the Process for notifying incidents to ONR (ONR-OL-PROC-002). Please note that individuals and/or organisations are responsible for complying with all their legal duties when notifying incidents to ONR.

The primary route for the submission of and access to incident information by dutyholders is the ONR dutyholder portal. Dutyholders who are unable to access the portal can provide the required information using an ONR Incident Notification Form (INF 1) for any safety, security, transport and safeguards incidents and submit this via email to onr.incidents@onr.gov.uk.  

In many circumstances, the dutyholder will be required to provide a follow-up report (FUR). A FUR template is provided here for this purpose.

RIDDOR incident notification

Incidents on a GB nuclear site, a new nuclear build site, an authorised defence site or any other defence nuclear site that are reportable under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) 2013 should be reported to ONR.

The ONR RIDDOR portal should be used for this purpose. Registered dutyholders with an online account should access the ONR RIDDOR portal from the ONR dutyholder portal.

General access to the ONR RIDDOR Portal is available here for those without an online account. There is no requirement to sign in or to register. Should the portal be unavailable, dutyholders can complete the RIDDOR reporting form and submit this to ONR via onr.incidents@onr.gov.uk.

Onward reporting of relevant information by ONR 

Dutyholders should note that the detail of some civil incidents reported to ONR will be shared with the Department for Energy Security and Net Zero (DESNZ). This is part of agreed ministerial reporting arrangements between ONR and DESNZ. Where incidents meet current Ministerial Reporting Criteria (MRC), which cover all five of ONR's purposes, details will be formally communicated by ONR to Ministers.

ONR publishes a quarterly statement of incidents that have met MRC on the ONR website.


Consignment/Shipment notification for transport of class 7 dangerous goods (radioactive material)

Provision of work related information

For work taking place on a GB nuclear site, a new nuclear build site, or an authorised site, dutyholders are expected to report or notify the following activities or information:

Asbestos notifications

Notifiable licensed work

Notifiable asbestos removal by a licensed asbestos contractor should be reported using the ASB5 online form via the HSE website within the notifiable period.

Notifiable non-licensed work (NNLW)

Sporadic and low intensity exposure not exceeding the control limit should be notified to ONR via the HSE online notifications form.

Construction project notification

In accordance with the requirements of the Construction (Design and Management) Regulations 2015 construction projects that will:

  1. last more than 30 days and have more than 20 workers working simultaneously at any point in the project; or
  2. exceed 500 person days,

should be notified to ONR via HSE's online system on a F10 'Notification of construction project' form.

Where limitations apply for provision through this method, the dutyholder should seek alternative agreed arrangements.

Reporting of statutory examination defects/Adverse Inspection Reports

Competent persons carrying out statutory examinations have a duty to report certain categories of defect under legislation.

The duty to report a defect is irrespective of whether that defect has been remedied following the examination equally applies to both of the following:

Lifting Operations and Lifting Equipment Regulations 1998 (LOLER)

Under LOLER, issues described by the following definition must be reported: "...a defect in the lifting equipment involving an existing or imminent risk of serious personal injury..."

Note: If the lifting equipment has been hired or leased by the employer, the HSE will be the relevant reporting authority rather than ONR and reports should be made to HSE.

Pressure Systems Safety Regulations 2000 (PSSR)

Under PSSR, issues described by the following definition must be reported: "...the pressure system or part of the pressure system will give rise to imminent danger unless certain repairs or modifications have been carried out or unless suitable changes to the operating conditions have been made." 

PSSR also requires notification of any agreed postponement of an examination planned under a written scheme.
