Aim of inspection

The purpose of the inspection was to review compliance of the the University Hospital of Wales (UHW) arrangements for class 7 dangerous goods (radioactive material) for road transport, covering consignee and consignor duties.

Key findings, inspector's opinions and reasons for judgement made

The inspector reviewed a range of topics and identified a number of examples of UHW meeting relevant good practice, in reducing risk to levels that are ALARP and demonstrating compliance with IRR17 and CDG09, including appropriate IRR17 consent and registration, appointment of Safety Advisers (Radiation Protection Advisers and a Dangerous Goods Safety Adviser), appointment and training of Radiation Protection Supervisors, transport documents, transport package certification, security, Dangerous Goods Safety Advisers report and management system.

 

The inspector identified two areas of non-compliance and discussed these during the inspection. These were associated with IRR17 transport radiation risk assessments and ADR consignor duties associated with return of spent quality assurance sealed sources to the supplier. 

Conclusion

The inspection findings were presented to UHW at the close of the inspection.
 

I judged an overall inspection rating of 'Green - No formal action' was appropriate for this inspection as as there were no significant shortfalls identified in the delivery of safety.

 
UHW is required to provide ONR either with evidence that the two identified non-compliances have been resolved, or an appropriate resolution plan with timescales for completion, by 27 September 2025. The non-compliances will be tracked to completion via a level 4 regulatory issue.
 

As UHW is part of Cardiff and Vale University Health Board, the findings of this inspection should be applied to all transport activities undertaken by the trust.
