Aim of inspection

The inspection was aligned to the ONR Decommissioning, Fuel and Waste strategy. The aim of the inspection was to conduct a one-day themed inspection relating to reporting culture and effective Operational Experience Feedback (OEF) including a compliance inspection of Licence Conditions 7 and 17. During the inspection, I sought evidence that: The Licensee has an active reporting culture, recording and investigating and trending events and identifying and implementing corrective actions to help prevent recurrence i.e. an effective OEF process. OEF information from events is shared between all relevant staff in Magnox Ltd and its contractors. Staff and managers actively pursue and address organisational learning opportunities from all relevant sources. The NDA "Organisational Learning" good practice guide is being implemented.

Key findings, inspector's opinions and reasons for judgement made

I carried out a themed LC7 and LC17 inspection at the Magnox Ltd Hinkley Point A site. I reviewed the arrangements for these LCs and was able to establish the “golden thread” in the management system for them. I utilised the recent event involving the reporting of incidents relating to INF-2409 at Magnox Ltd Hinkley Point A to examine the reporting processes and culture from both Magnox Ltd staff and its contractors. I assessed any initiatives to improve cultural reporting from both Magnox Ltd staff and its contractors.

Conclusion

In my opinion, the Licensee's arrangements for compliance with LC7 and LC17 are adequate. It was my opinion the initiatives to improve cultural reporting from both Magnox Ltd staff and its contractors have been largely implemented and adequate progress is being made to close ONR regulatory issue 11133 used to track the enforcement letter, ONR-EL-22-03, actions.  I therefore concluded that this inspection identified no further matters that may impact significantly on nuclear safety.
