Aim of inspection
The scope of the intervention was aligned to the ONR Decommissioning, Fuel and Waste strategy. The aim of the inspection was to conduct a one-day themed inspection relating to reporting culture and effective Operational Experience Feedback (OEF) including a compliance inspection of Licence Conditions 7 and 17.
During the inspection, I sought evidence that:
- The Licensee has an active reporting culture, recording and investigating and trending events and identifying and implementing corrective actions to help prevent recurrence i.e. an effective OEF process.
- OEF information from events is shared between all relevant staff in Magnox Ltd.
- Staff and managers actively pursue and address organisational learning opportunities from all relevant sources.

Key findings, inspector's opinions and reasons for judgement made
I carried out a themed LC7 and LC17 inspection at the Magnox Ltd Oldbury site. I reviewed the arrangements for these LCs and was able to establish the “golden thread” in the management system for them. I utilised an incident at Magnox Ltd Oldbury (hand injury) to examine the reporting processes to external agencies, the terminology used and governance arrangements prior to sending the information from the site. I assessed the adequacy of the internal site investigation. I asked if an adequate risk assessment was in place following the event and how this had been updated as well as any operating or maintenance instruction post-event. I was informed the use of doors at Magnox Ltd Oldbury was still subject to a dynamic risk assessment by staff using them, which remains appropriate, and similar doors had been subjected to assessment of risk and their use sanctioned appropriately. It is my opinion this meets the requirements of the Management of Health and Safety at Work Regulations 1999 (MHSWR). Following a recent set of events, mostly relating to setting-to-work, I assessed the setting-to-work brief given for work occurring on the day of the inspection. I then attended the location of the work and was assured the operators had been given appropriate instructions for the work being undertaken.
Conclusion
In my opinion, the Licensee's arrangements for compliance with LC7 and LC17 are adequate. My assessment of the setting-to-work brief given for work occurring on the day of the inspection was in my opinion adequate and appropriate to the task I inspected. I therefore concluded that this inspection identified no further matters that may impact significantly on nuclear safety.
