Executive summary

Purpose of intervention

To undertake a compliance inspection of LC 7 (Incidents on site) arrangements in Waste and Effluent Disposition Disposal (WEDD), Highly Active Liquor Evaporation and Storage (HALES) and Waste Vitrification Plant (WVP) facilities.

Key Findings, Inspector's Opinions and Reasons for Judgements Made

It was found in these facilities, Sellafield Limited is adopting and implementing its own arrangements for compliance against LC7 (SLSP 3.09.100) and that the sentencing of events is timely and the depth of investigations appeared proportionate. However, further work is required to fully embed compliance with the site procedures and supporting guidance and to encourage all operators to fully embrace the event reporting system.

Although there is a process in place for managing actions arising from condition reports the numbers of longstanding and overdue actions was greater than expected and is not regarded as good practice.

Sellafield Limited is now implementing tools to facilitate the trending of information from events and this will be utilised by the Directorate Operating Experience (OPEX) team to assist the Operating Units (OU) in developing effective corrective and preventative actions to reduce the number of future events and to minimise repeat events.

Conclusion of Intervention

The adoption and application of site-wide LC 7 arrangements for event reporting, sentencing, investigation and management of corrective and preventive actions is judged overall as adequate. However, there is scope for improvement with regard to management of longstanding and overdue corrective actions, management of extensions to delivery dates of actions and the need for continued encouragement to all operators to utilise the event reporting system.
