Executive summary

Purpose of Intervention

In accordance with Devonport Royal Dockyard Limited (DRDL) 2016/17 intervention plan and propulsion sub-programme strategy, a number of routine interventions were conducted. Follow up enquiries relating to two events, and readiness meetings associated with projects were also conducted.  

Interventions Carried Out by ONR

Follow up enquiries were conducted following two reportable events which occurred on the Devonport site. Discussions were held with a number of key stakeholders associated with the event to gain a better understanding of what happened and the actions DRDL are putting in place to prevent a repeat occurrence.

Routine plant management meetings were held to provide progress updates on the 9 dock facility. Discussions were held around safety issues, plant status and project progress.

ONR sampled evidence relating to the readiness to proceed with refuel activities on the Devonport site. ONR held meetings with the plant management and internal regulator, ONR sampled a number of areas to ensure the effectiveness of the readiness review process.

Key Findings, Inspector's Opinions and Reasons for Judgements Made

From discussions held with key stakeholders and the evidence examined relating to the two reportable events, ONR are content that DRDL have taken the appropriate remedial action in response to the events. Initial learning from the Operational Experience Feedback (OEF) investigation has been acted upon and improvements have been made in the short term. DRDL have initiated internal investigations into the events which ONR will follow up on during future interventions.

Following discussions with the plant management team ONR was satisfied that DRDL are managing the 9 dock activities adequately.

ONR identified a number of areas for improvement relating to the operational readiness review process. As the current process was implemented a number of years ago, DRDL acknowledged that it should be reviewed to ensure  alignment with other supporting processes and that it is still fit for purpose.  

Conclusion of Intervention

We judged the licensee has adequately implemented the interim improvements relating to the events on the DRDL site. Internal investigations are currently underway and ONR will follow up on the associated actions and recommendations to ensure long term improvements

ONR are content with DRDL’s strategy for conducting operational readiness reviews, however DRDL have acknowledged that a review of the operational readiness review process is required.

Initial feedback was provided during the interventions with respect to our findings and ONR will follow up on these issues during routine interventions.
