This report covers inspections carried out as part of ONR's regulation of operations undertaken at the Cyclife Metal Recycling Facility.
A Systems Based Inspection (SBI) covering Essential supplies e.g. Electrical, Comp. Air, Inert Gas) was undertaken, and other meetings were held with Safety Representatives and to review regulatory Issues, and a discussion on some general matters identified during previous site visits were carried out.
During my inspections at Cyclife I did not identify any safety significant matters requiring formal action from the Licensee. However, I did make the observations recorded below.
Investigations undertaken by Cyclife into Events need to be more thorough when looking to identify Event Root Causes. Sufficient "whys" need to be asked to identify actions that will prevent the Event from recurring.
Cyclife should consider within their Safety Case the safety implications from personal protective equipment removal following C3 operations in an unventilated area within the facility.
Cyclife should review their maintenance procedures with a view to employing end-to-end testing of safety systems where practicable.
Cyclife should review their overdue list of items on their maintenance management system to ensure that there is no adverse impact of missed maintenance on safety systems.
Cyclife should establish clear boundaries to their ownership of the electrical supply system to their site and record this within their Safety Case.
Cyclife should endeavour to make learning available to personnel from external Events so that appropriate lessons may be applied when applicable.
I consider that observations made during this inspection were minor in nature and that the safety implications of findings from the SBI are being adequately addressed by Cyclife.
