Executive summary

Purpose of intervention

Follow up inspection to an event that was reported to ONR by GE Healthcare Cardiff Nuclear Licenced Site.   Waste was despatched with some of the packages not being on the shipping paperwork and not being adequately labelled.

A follow up inspection of the licensee’s investigation, the circumstances leading up to the event, and the changes made by the licensee to prevent a recurrence.

Key Findings, Inspector's Opinions and Reasons for Judgements Made

The licensee had estimated the volume of solid Low Level Waste (LLW) that would be generated during the decommissioning of the Liquid Handling Centre (LHC) but once work started the quantity was significantly higher.  An increased number of drums and lack of clear segregation and control made it possible to load the wrong drums onto a trailer for despatch.  The licensee has now redesigned and improved the waste handling and clearly segregated the drums.  Additional checks are in place producing more robust control over the waste.

Conclusion of Intervention

ONR is satisfied that the duty holder has carried out a thorough investigation into the causes of the event.  A number of improvements have been implemented to enhance controls to prevent a recurrence.  No further action is required by ONR.
